• Souhegan Valley Sabers Application 2012 (1)
• Saber Physical Examination and  Medical History Form (2)
• Saber Medical Release Form (3)
• Saber Volunteer Signup Form (4)
• Saber Code of Ethics Form (5)
• Submit copy of Birth Certificate
• Submit copy of 2011-2012 report card

 

NOTE: Successful registration is defined as all required paperwork submitted and filled out accurately and payment received by the Souhegan Valley Sabers no later than August 1, 2012.
